General Registration for New and Current Patients

If your child is a new patient and has not completed the Diagnostic Evaluation, Child Observation and Parent Training Session, and Parent Feedback Session, then select the button below to Pay Registration and the Intake Evaluation Fee button below. If your child is a current patient and has already completed the intake evaluation process within the last six months, select the Pay Registration Fee button below.

Payment instructions:

1.) In the "Notes" section, please provide your child's name, age, and grade they will be in for Fall 2024. 

2.) You will receive an automated receipt once payment is processed.

3.) Our Intake Coordinator will contact you and provide you with the registration forms to complete through our clinic portal. This follow-up will occur during our business hours 9am-5:30pm Tues - Saturday. (We are closed on Mondays.)
